NOTE: The due date has been EXTENDED
to TUESDAY, OCT. 17, for the Benefit Form.
2 – WADA
Workshop Registration Form
~ send this form to Joe
Beran at La Crosse Central H.S.
~ due before
Friday, Oct. 13… to avoid a late fee.
You MUST send in BOTH forms… one form to register for the Workshop
and the other to receive a WADA membership benefit gift at the event.
If you have not yet downloaded the on-line

do as soon as possible.
Remember, the registration forms will not be
mailed, as was done in past years.
